Here’s something I found at a small commercial site recently.
The exhaust fan shown was originally intended to discharge stale air to outdoors via a gooseneck on the rooftop.
The flex duct was not properly fastened to the discharge duct. Over time the fan simply blew the flex duct off the hard pipe allowing stale air to be discharged into the ceiling space and then recirculated back into the occupied space.
